  Hot Zone Algorithm SoftCare uses advanced dispatching techniques to define Hot Zones. Icons and metaphors provide immediate feedback from the Hot Zone List (e.g. – the phone icon means to call the family, red stoplight means immediate attention is needed). The Hot Zone optimizes the medical professional's time and identifies the most important work.

  Automated HIPAA Compliance SoftCare records every HIPAA compliant letter that is mailed and then tracks responses in order to determine the appropriate follow-up.

  Mail / Call Algorithm SoftCare performs an analysis of activity at the Health Center every night. This analysis records patient intake and places patients in queue for follow up contact via email, automated patient dialing or regular mail.   Patient Profile Quickly identify the next step with every patient. The Patient Profile provides a 360-degree view of every patient. It summarizes recommended treatments, treatments rendered, responsible parties, notes, assigned tasks, treatment status, payments and the next steps all in one view. Instantly "Drill Down" into any information in the Profile with a single click.

  Family Involvement SoftCare identifies family members so that the medical team can quickly contact the proper parties for treatment authorization. The elderly in long term care facilities often have more than one family member involved in the authorization of their care.

  Patient Team Care Define all the people involved in providing care to each patient along with their respective roles: health counselor, primary physician, dentist, hygienist and social worker.

  Patient History The Patient Notes section provide a complete history of all patient and/or Medical Team activity. Everything is recorded, from the mailing of HIPAA compliance letters to communication with family members.

  Automated Task List Every member of the medical care team knows exactly what they need to do and when they need to do it because the Task List (called Activities) are displayed in the Patient Profile. Every member of the Medical Team also views their own personal "to-do" list when they login. This means that no task on the list goes un-noticed.

  Automated Treatment Notification All treatments are listed in the Patient Profile. Every treatment has a specific status: Recommend, Pending, Released or Complete. Recommended treatments are automatically sent to the Health Professional who’s job is to obtain approval from the Responsible Party. After authorization and payment is received, the treatment is Released and automatically sent to a specific Practitioner depending on the type of treatment. When the treatment is completed, the system automatically notifies the Responsible Party and issues payment to the Medical Team.

  Reminder Management A family member may agree to pay for treatment and put a check in the mail. However, they may forget to send payment. SoftCare tracks the receipt of payment and automatically mails out notifications if payment is not received. The Reminder system is vital to making sure that treatment is delivered in a timely fashion and never drops through the cracks.

  One Degree of Separation There is only "One Degree of Separation" from important data when you are in the Patient Profile. If you need the phone number or email of a Practitioner – one click. If you need to contact a family member – one click. If you need to find out the details of a treatment – one click.   The Patient Pipeline
Coordinating patient activities from initial contact to treatment delivery is a critical part of the running the business and managing patient relationships. HCMS’s pipeline management helps keep your Health Counselors organized and focused to make sure your patients are receiving the attention they need.
  Pipeline Workflow SoftCare automatically creates tasks as a patient moves through the pipeline. Each task is dynamically assigned to the right person based on role such as Physician, Health Counselor, Dentist or Hygienist.

  HIPAA Compliance SoftCare records every HIPAA compliant letter that is mailed and then tracks responses in order to determine the appropriate follow-up.

  Online Mail Merge All letters are online and printed via a scalable thin client. This means that database merging is done automatically.

  Facility Specific Mailing Customized letters are sent based on the patient and facility. Every letter is personalized.

  Patient Specific Different patients have different profiles such as those that live in assisted living facilities (ALF) vs. special needs facility (SNF). SoftCare matches direct mail with various patient profiles.

SoftCare provides intelligent dialing. Automatically analyzes busy signals, ring tones, phone number disconnects, answering machines or voice mail. Finds only live parties and connects them to a free Health Counselor.

  Architecture

The FTI Dialer uses a state of the art Intel hardware that employs a CT Bus using TDM (Time Division Multiplexing) to convert the signal from analog to digital. The digital signal is then passed to the PCI Bus of the computer which then hands it to the the FTI Dialer Application.

The FTI Dialer senses the line and monitors the database in real time to detect changes, such as when a Health Counselor becomes active. To say the least, the dialer is very busy monitoring and reacting to the changes as they occur. This is a complex dynamic environment.

  Kwik Link Kwik Link is the communications program that transfers patient information between the Dialer and the national SoftCare database. It is an encrypted and authenticated connection through the Internet. It is the fastest known program of it’s type transferring as many as 2,000 records in one second. The Kwik Link technology was developed by FTI Global.

  ADIX Monitor

The ADIX Monitor is program that monitors all events that occur in the ADIX switch, even lifting a handset from a cradle is reported to the ADIX Monitor. This determines if a station is busy or idle. The ADIX Monitor is installed on the FTI Dialerserver.

The ADIX Monitor listens for an extension to become free. The station has a specific station number that begins with an "S" and is stored in the kwik_link.mdb database. This information is then provided to the Dialer application to determine the "state" of the station.
